1. When the Official Form Is Required
Under current Ala. R. Civ. P. 3(b):
☐ Initial circuit-court complaint, not domestic relations. File the complaint with a completed and signed Form 93.
☐ Appeal to circuit court from district court. File the notice of appeal with a completed and signed Form 93.
☐ Appeal to circuit court from probate court. File the notice of appeal with a completed and signed Form 93.
☐ Not this form. Domestic-relations matter, initial district-court action, or another filing outside Rule 3(b)'s Form 93 requirement. Rule 3(dc) applies Rule 3 in district court except for Rule 3(b)(1).
If a required cover sheet is omitted, Rule 3(b)(3) says the omission does not invalidate commencement of the case or appeal. The clerk must accept the complaint or notice of appeal and notify the filer; the filer must promptly supply the sheet. The court may later stay or, after proper notice, dismiss, treat the failure as contempt, or issue another appropriate order.
