MISSOURI GENERAL CIVIL CASE COVER SHEET

FI-50 Confidential Filing Information — Preparation Companion

NOT THE OFFICIAL FORM — DO NOT FILE. Missouri Courts currently uses
Form FI-50 for probate and non-domestic-relations matters. This companion
helps collect and verify the inputs without reproducing confidential personal
identifiers in an ordinary public cover sheet.

1. Current Form and Filing Gate

Court Operating Rule 4.07.1 requires the filing information sheet in courts
using the statewide-approved automated case-management system. Rule 4.07.2
allows other courts to require it. Under Rule 4.07.1(6), a separate sheet is
not required for an electronic filing if the required party information is
contained in the electronic submission. Confirm the actual submission and
clerk instruction; do not assume the exception.

2. Case-Level FI-50 Inputs

Use only a code and description from the current official Case Types List.
The former template's home-grown contract, tort, equity, mechanic's-lien,
associate-civil, and small-claims checklist is not a substitute for that list.

3. Party Input Register

Complete one row for every party known when filing. Keep actual sensitive
identifiers in the secure system used to complete FI-50, not in this companion.

For a person, the current FI-50 asks for name, address and contact information,
date of birth, sex, Social Security number, race and ethnicity entries and
source, and counsel information. It states that the full Social Security
number is required if known and reasonably available under Rule 4.07.

For an organization, use the current organization and party fields on FI-50.
Do not invent birth, sex, race, ethnicity, or Social Security information for
an entity.

4. Confidentiality and Redaction Separation

Rule 4.07 treats filing information sheets as confidential records. Form FI-50
states that it is for court use only. Do not attach FI-50 to a public-facing
pleading as an ordinary exhibit or copy its confidential data into this
companion.

The Missouri Courts filing-information page separately discusses confidential
redacted-information filings. Determine those requirements from the current
rules and filing page; FI-50 does not replace a redaction certificate or
another required confidential filing.

5. Items the Former Cover Sheet Improperly Combined

Prepare these, if needed, under their own current rule, official form, and
local instruction:

This companion contains no dollar threshold, filing fee, service preference,
jury election, or attorney certification. Those issues are not FI-50 fields.

6. Final Transfer to Official Form

☐ Current FI-50 opened in the court-supported software or printed for permitted
paper completion

☐ Case Type Code and description copied from the current official list

☐ Each Party Type Code and description copied from the current official list

☐ Each party known at filing included

☐ Confidential identifiers transferred from the secure source directly to
FI-50 or the authorized eFiling fields

☐ Submitter and page-count fields completed

☐ Entries reviewed for completeness and accuracy

☐ Filing route, confidentiality level, and any local additions confirmed with
the circuit clerk

☐ This preparation companion remains outside the filed record

7. Completion Record

This packet is complete only as an FI-50 preparation record. File the current
official form or supply the authorized electronic fields, not this document.
