2. MAINE ROUTE GATE
For § 1593 reinstatement, state the name, effective dissolution date, that the grounds did not exist or were eliminated, and that the name complies with § 1508. Failure to reinstate within six years results in cancellation on the sixth anniversary.
For § 1604 revival, an interested party must identify the former and proposed names, formation/dissolution/cancellation dates, prior agent, requesting parties, specific purpose, and time needed. The Secretary of State decides whether the company should be revived for that purpose and period.
