Supporting information and privacy
Select one:
☐ I enclose information from my medical provider about the disability-related need for this animal. Enclosure: [DOCUMENT DESCRIPTION AND DATE].
☐ I can supply reliable information if needed to evaluate this request. Please identify the particular information needed and a private way to send it.
☐ My disability and my disability-related need for this animal are readily apparent or already known to the housing provider. I ask that you evaluate the request on the information already available.
When a landlord is asked to make an exception to a policy prohibiting animals, Ala. Code § 24-8A-3(a) permits documentation requests about a disability or disability-related need only when that fact is not readily apparent or known to the landlord. The state chapter defines reliable documentation in § 24-8A-2(4). Please keep any documentation obtained under § 24-8A-3 confidential as subsection (b) requires.
If you need further information or see a concern about the proposed accommodation, please tell me what is needed so we can discuss a workable solution.

Landlord-tenant paperwork governs who can stay in a property, on what terms, and what happens when something goes wrong. Leases, notices to quit, security deposit demands, and habitability complaints all have state and often city-specific requirements for timing, content, and service. Getting the paperwork right is what makes an eviction actually succeed or a security deposit actually come back, because judges regularly dismiss cases over small procedural mistakes.
