Illinois Assistance-Animal Accommodation Request and Decision Packet
ILLINOIS ASSISTANCE-ANIMAL ACCOMMODATION REQUEST AND DECISION PACKET
ILLINOIS RULES
The Assistance Animal Integrity Act permits reliable documentation only when disability or disability-related need is not readily apparent or known. Documentation must be written, come from a person with a therapeutic relationship, and describe the disability-related need. A provider may offer a form but may not deny because it was not used.
The provider may request separate support for each animal and may verify authenticity. It may not require a pet deposit, pet fee, pet assessment, or special liability insurance. Damage may be charged on the same basis as for other residents, ordinary wear excepted. Denial or rescission requires an undue financial or administrative burden or an individualized direct threat that reasonable measures cannot reduce or eliminate.

About This Template
Landlord-tenant paperwork governs who can stay in a property, on what terms, and what happens when something goes wrong. Leases, notices to quit, security deposit demands, and habitability complaints all have state and often city-specific requirements for timing, content, and service. Getting the paperwork right is what makes an eviction actually succeed or a security deposit actually come back, because judges regularly dismiss cases over small procedural mistakes.
