4. Statutory Notice Provisions
The undersigned Trustee hereby provides the following notice pursuant to A.R.S. § 14-10813:
4.1 Notice of Trustee Identity (A.R.S. § 14-10813(B)(2))
The Trustee identified in Section 2 has accepted the trusteeship of the Trust identified in Section 1. The Trustee's name, address, and telephone number for purposes of all communications relating to the Trust are set forth in Section 2 above.
4.2 Notice of Existence of Irrevocable Trust (A.R.S. § 14-10813(B)(3))
You are hereby notified that:
(a) The Trust identified in Section 1 exists and is presently irrevocable;
(b) The identity of the Settlor(s) is set forth in Section 1;
(c) You have the right to request from the Trustee a complete copy of the trust instrument, including all amendments; and
(d) You have the right to a trustee's report under A.R.S. § 14-10813(C), which will be furnished at least annually and at the termination of the Trust, and which will include the trust property, liabilities, receipts, disbursements, a listing of trust assets (with market values if feasible), and the source and amount of the trustee's and any agent's compensation.
4.3 Right to Request Trust Terms
Upon written request directed to the Trustee at the address in Section 2, the Trustee will furnish to you, without charge, a complete copy of the trust instrument and all amendments thereto.
4.4 Trust Protector (if applicable)
A Trust Protector serves with the powers set forth in A.R.S. § 14-10818 and the trust instrument:

5. Right to Object; Limitations
You may communicate with the Trustee at any time concerning the administration of the Trust. Certain limitations periods may begin to run upon delivery of trustee reports or other adequate disclosure under A.R.S. § 14-11005. 6. Waiver (A.R.S. § 14-10813(E))
