2. APPLICATION AND TAX-CERTIFICATE GATE
The application states the corporation's name and effective dissolution date, that each ground did not exist or has been eliminated, and that the name satisfies RSA 293-A:4.01.
If the Secretary of State receives the application more than 120 days after the dissolution notice was mailed, the application must contain the Department of Revenue Administration certificate required by RSA 77-A:18, III and RSA 77-E:12, III.
