2. SECTION 29-106.03 APPLICATION
The application must be signed by the entity and state the name at administrative dissolution and, if needed, a different name satisfying § 29-103.01; principal-office address; registered-agent name and address; dissolution effective date; and that the grounds did not exist or have been cured.

4. EFFECT AND RELIANCE REVIEW
Under § 29-106.03(d), reinstatement relates back to the administrative-dissolution effective date and the entity resumes its activities and affairs as if dissolution had never occurred.
