Oregon Circuit Court Civil Cover Sheet
OREGON CIRCUIT COURT — CIVIL COVER SHEET
For Use in All 36 Oregon Circuit Courts
IMPORTANT FILING NOTES
Oregon circuit courts require a civil cover sheet or case information sheet to accompany the initial filing in civil cases. The Uniform Trial Court Rules (UTCR) govern statewide practice, but individual counties may impose Supplementary Local Rules (SLRs) that add requirements such as arbitration track designation, business court assignment, or additional case information forms. Filers must verify current county-specific requirements before filing.
E-Filing: Oregon circuit courts use mandatory electronic filing under UTCR Chapter 21. All documents must comply with UTCR 2.010 formatting standards and the Oregon Judicial Department's eFiling Standards guidance document. Paper filing is permitted only in limited circumstances authorized by UTCR 21.040.

This template is designed as a worksheet to assist in completing the official Oregon circuit court civil cover sheet or case information form required by the applicable county. Always verify requirements with the specific county clerk's office and review current UTCR and SLR provisions before filing. Filing fees and procedures are subject to change.
About This Template
These are the filings that drive a lawsuit through the system: complaints, answers, motions, briefs, discovery requests and responses, and post-judgment papers. Each has its own format requirements under federal and state procedural rules, and each has a deadline that cannot be missed without consequences. Clean, procedurally correct filings move a case forward; sloppy ones invite motions to strike, amended responses, and avoidable delays.
