organizing the confidential party information transferred to current Form
FI-50 or authorized Missouri eFiling fields. It does not decide the claim,
division, venue, amount in controversy, service, jury demand, fee, or remedy.

1. Form-Control Record

Rule 4.07.1(6) addresses electronic filings that already contain the required
party information. Confirm that the actual submission satisfies the rule
before omitting FI-50.

2. Case Fields

Use the current official case-type code and description. Do not rely on the
former template's “CC” and “AC” checklists, dollar bands, or generic subject
labels.

3. Secure Party Index

Do not enter a Social Security number, full birth date, confidential address,
or other protected identifier in this companion. Record only the approved
secure-source location used for direct transfer to FI-50.
