6. REQUIRED SUBMISSION PACKAGE

7. SIGNATURE
The signer must check “I ACCEPT,” sign, print the signer's name, date the filing, and select one authorized capacity.

8. AFTER APPROVAL
8.1 Publication or ACC Database Entry
A.R.S. § 10-1503(D) requires that, within 60 days after approval, either a copy of the application be published or the ACC enter the approval information in its statutory database. Do not publish before approval. Follow the approval letter's instructions and retain proof of the completed route; an affidavit of publication may be filed if publication applies.
8.2 Amendments and Restatements
Within 60 days after an amendment or merger restatement becomes effective, deliver an authenticated copy to the ACC. File Articles of Amendment to Application for Authority if the corporation changes its actual or Arizona name, duration, or domicile, or if a statement in the original application was inaccurate when made.
8.3 Annual Report
File the corporation's annual report and pay the annual fee by the date assigned by the ACC. Later reports are due in the anniversary month on the date determined by the ACC. A written extension request of up to six months must be filed on or before the due date with the annual registration fee. Confirm the assigned date in the ACC entity record.
