FILING INSTRUCTIONS
This Civil Case Information Sheet must accompany the filing party's first appearance in any civil case filed in Idaho District Court, as required by Idaho Rules of Civil Procedure Rule 3(d)(1). The form is approved by the Idaho Supreme Court. All parties must file a completed case information sheet with their first appearance, with the exception of governmental entities and certain specialized case types.
Key Requirements:
- File this sheet with the Complaint or Petition (plaintiff/petitioner) or with the first responsive pleading (defendant/respondent)
- Use the official Idaho Supreme Court-approved form for actual filing
- Confidential party identifiers (SSN, DOB, driver's license) must be filed on the separate Confidential Case Information Sheet pursuant to Idaho Court Administrative Rule 32
- Electronic filing is available through the Idaho statewide e-filing system under I.R.E.F.S.

Mandatory Filing: Under I.R.C.P. Rule 3(d)(1), the Civil Case Information Sheet must accompany every party's first filing. The clerk may reject a filing that does not include a completed information sheet.

Governmental Entity Exception: Governmental entities are generally exempt from filing the case information sheet in certain categories.

Medical Malpractice Pre-Litigation Requirements: Under Idaho Code § 6-1001 et seq. (Idaho Medical Malpractice Act), medical malpractice claims require a pre-litigation screening panel hearing before filing suit, unless the parties agree to waive the requirement.

Idaho Long-Arm Statute: Service on out-of-state defendants may be accomplished pursuant to Idaho Code § 5-514 by serving the Secretary of State.

Venue: Venue in Idaho civil actions is governed by Idaho Code § 5-401 et seq. Actions must generally be filed in the county where the defendant resides or where the cause of action arose.
