COMPLAINT FOR DIVORCE

NOW COMES Plaintiff, [_______________________________________], and for cause of action against Defendant states:

9. Pension / Retirement. Any rights of either party in the other's pension, annuity, or retirement benefits are governed by the Settlement Agreement and any required Eligible Domestic Relations Order (EDRO) / Qualified Domestic Relations Order (QDRO).

10. Insurance Beneficiary Status (MCL § 552.101). Plaintiff requests that all rights of the parties in any policy or contract of life, endowment, or annuity insurance of the other be determined consistent with the Settlement Agreement.

11. Servicemember. Upon information and belief, Defendant is not entitled to the protections of the Servicemembers Civil Relief Act.

TAB 13 — JUDGMENT OF DIVORCE

This matter came before the Court for entry of a Judgment of Divorce after expiration of the statutory waiting period under MCL § 552.9f (or § 552.9g) on [__/__/____], based upon the pleadings, the parties' Marital Settlement Agreement, the Uniform Child Support Order, the FOC recommendation, and (where applicable) the testimony of Plaintiff at proof hearing.
