II. RECITALS AND AUTHORITY TO AMEND

WHEREAS, the Settlor established the above-referenced revocable living trust (hereinafter referred to as "the Trust") on [__/__/____] under the laws of the Commonwealth of Kentucky;

VI. CONFIRMATION OF UNAMENDED PROVISIONS

IX. SEVERABILITY
