AMENDMENT NO. [NUMBER] TO [AGREEMENT TITLE]

3. GOVERNING LAW AND FORUM

• Except as amended in Section 1, the governing law and forum provisions in the Agreement remain unchanged.
• If the Agreement is silent on governing law or forum, this Amendment and the Agreement (as amended) are governed by the laws of the State of Alabama, without regard to conflict-of-law rules, and the state and federal courts located in [COUNTY], Alabama have exclusive jurisdiction.

4. NO OTHER CHANGES

• Except as expressly amended, the Agreement remains unchanged and in full force.

5. COUNTERPARTS; EXECUTION

• This Amendment may be executed in counterparts and by electronic signatures; all counterparts form one instrument.

6. EFFECTIVE DATE

• This Amendment is effective on [DATE] (or upon last signature if left blank).

7. REAFFIRMATION

• Each party reaffirms its obligations under the Agreement as amended.
