1. PARTIES

3. FACTS COMMON TO ALL CLAIMS

3.1 The Contract. On or about [DATE], Plaintiff and Defendant entered into a written agreement titled “[TITLE OF AGREEMENT]” (the “Contract”), a true and correct copy of which is attached hereto as Exhibit A.

3.2 Plaintiff’s Performance. Plaintiff fully performed, or was ready, willing, and able to perform, all of its obligations under the Contract, including but not limited to:

4. COUNT I – BREACH OF CONTRACT

5. DAMAGES
