I. DOCUMENT HEADER

1.1 Declarant. I, [NAME OF SERVER] (“Declarant”), declare under penalty of perjury that the following is true and correct.
1.2 Effective Date. This Affidavit of Service (“Affidavit”) is made effective as of [DATE] in the State of Washington.

8.2 Forum Selection. Any dispute concerning the validity, sufficiency, or admissibility of this Affidavit shall be determined exclusively by the Superior Court of Washington for [COUNTY] County.

IX. GENERAL PROVISIONS
