1. IDENTIFICATION OF TESTATOR AND UNDERLYING WILL

7. ATTESTATION CLAUSE

8. OPTIONAL SELF-PROOF REVIEW

RSA 551:2-a(I) provides an oath and acknowledgment form for a self-proved will. It requires the testator's and witnesses' signatures to be followed by a sworn acknowledgment before an authorized officer. The statute's printed form says “will”; this packet does not substitute an edited codicil oath and assume it qualifies. If self-proof is desired for this codicil, have New Hampshire probate counsel confirm the applicable route and prepare the corresponding oath before the execution ceremony.
