2. JURISDICTION AND VENUE
2.1. 4. GROUNDS FOR MODIFICATION
4.1. Pursuant to IC 31-16-8-1, Petitioner requests modification of the Existing Order on the following grounds (check all that apply):

Modification Standard
Under IC 31-16-8-1, the petitioner must demonstrate either:
1. A substantial and continuing change in circumstances since the last order, OR
2. A 20% or greater deviation between the existing order and the amount that would result from current application of the Guidelines (if 12+ months have passed)
